Adding or Removing a Family Member From Health Coverage
Send Enrollment/Change form to Employee Benefits within 31 days of event!
Whenever you have a qualified family status change you need to notify Employee Benefits within 31 days of the date of the event to update your medical and dental coverage. Otherwise you will need to wait until open enrollment in November, effective January of the following year.
When removing a dependent (e.g. divorce or a child no longer eligible) notification must be made within 60 days or they may lose their right to continuation coverage through COBRA benefits.
A qualified family status change is one of the following:
Birth or adoption of a child When adding an adopted child, please include photocopy of adoption papers.
Marriage; eligible step children may be added at this time When adding a spouse, please include a photocopy of your marriage license.
Domestic partner Eligibility after 1-year anniversary of living together; or Within 31 days of registering as an Oregon Domestic Partner. Please provide copy of Certificate of Registration. Loss of medical coverage for an eligible family member not currently on Marion County health plan
Dependent child turns 26 years of age
Death of a benefit covered family member Divorce
