FLEXIBLE SPENDING ACCOUNT - HEALTHCARE AND DEPENDENT CARE
These accounts allow you to use pre-tax dollars to pay for out-of-pocket medical, vision, prescription, and dental expenses, employee health insurance premiums, and dependent care expenses that you would normally have to pay with after-tax dollars. You are eligible for FSA benefits when you are eligible for medical benefits. Participation in this plan is voluntary, and you can choose the account(s) in which you would like to participate.

FSA - Enrollment Form
- Enrollment is only during open enrollment for the following tax year, unless for a qualified status change. Status changes include:
Marriage
Divorce
Adding or deleting a dependent
Job status change
Starting or stopping daycare (for dependent care account)

COMMUTER EXPENSE REIMBURSEMENT ACCOUNTS (CERA)
These accounts allow you to use pre-tax dollars to pay for transportation and parking expenses that you would normally have to pay with after-tax dollars, such as van pool, mass transit, and parking in either a Marion County parking structure or other parking structure or lot. The CERA may be started or stopped on the first of the month at any time during the year.

Submitting A Claim
Complete a
Commuter Expense Reimbursement Account (CERA) Claim Form
and attach an itemized copy of the bill from your provider or an explanation of benefits (EOB) from your insurance company. Send the completed claim form and supporting documentation to BenefitHelp Solutions - PO BOX 67230, Portland, OR 97268-1240. Completed claim forms may also be faxed to 1-888-249-5058.
