3. Complete this form prior to disposal:
Oregon Department of Environmental Quality's Asbestos Shipment Report Form (ASN-4)

Asbestos Containing Waste (ACW): floor tile, cement board/siding, and composition roofing are common examples of asbestos containing waste, which were formerly referred to as non-friable asbestos. Friable asbestos fibers can more easily escape into the air. Examples are sheet vinyl with a paper or felt backing, pipe and boiler insulation (chalky blocked or gray cardboard type), duct insulation (a continuous wrap of gray paper on duct or gray tape on duct joints), and decorative sprayed-on materials (commonly known as popcorn ceiling).
