Case Management
Marion County Service Coordinators (also known as Case Managers) assist developmental disability clients who are eligible, and desire to have services, in getting their needs met. Case Managers are responsible for monitoring a client's services in residential sites and those clients who have in-home support plans. Specifically, Case Managers work to ensure that the Individual Support Plan is being followed, there are no health and safety issues, medical issues are being addressed and a person's money is being used appropriately, in accordance with the standards described in OAR 411-320-0130.
Marion County Developmental Disability clients will have an annual plan that addresses issues of independence, integration, and productivity. Marion County case managers are available to provide or arrange for Comprehensive In-Home Supports for Adults or Children and Family Supports, as required, to meet the support needs of eligible clients.
Case Managers may attend Individual Education Plan (IEP) meetings for children, assist in critical non-educational services for children, and participate in transition planning meetings of students 16 years of age or older.
