Case Management and Care Coordination

HIV Medical Case Management:

A service that links clients living with HIV to medical care, medications, and social services.

Services provided by an HIV Certified, Registered Nurse.

Nurse Case Manager does nursing evaluations, educates, and promotes a healthy way of living.

HIV Care Coordination:

Helps clients living with HIV get social services to meet their needs such as housing, transportation, food, as well as alcohol and drug treatment programs, mental health services, job programs and SSI/SSDI.

Helps clients to get these services through referrals, and by providing additional support when needed.
