The Choice Model helps people who have serious, long‑lasting mental health problems and need a lot of support. These people may:
- Be close to going to the Oregon State Hospital (OSH)
- Already be staying at OSH
- Have stayed in a mental health hospital in their town
- Live in treatment homes or group homes
Choice Model services:
- Help use hospital and treatment beds in a smart way
- Try to stop very long stays in hospitals
- Help people live as independently as they can
- Help doctors, counselors, and other helpers work together
- Make sure people get good care that fits their needs in their own community, so they can work on getting better where they live
Services Provided Under the Choice Model
We give help so people can live safely in their communities. Our services include:
- Planning moves from hospitals and care homes – We help people get ready to leave the Oregon State Hospital, other mental health hospitals, or care homes and move back into the community.
- Mental health help at home and nearby – We give counseling and other support where people live, not just in hospitals.
- Recovery services – We use programs like Assertive Community Treatment, where a team checks on people often and helps them with daily life.
- Supportive housing – We help people find and keep safe, stable places to live, with support when they need it.
- Peer support – People who have had mental health problems themselves help others by listening, sharing their stories, and giving hope.
- Removing roadblocks to community living – We help people get the services and support they need, even if other programs do not pay for them, so they can live well in their community.
Eligibility for Enrollment
A person can get Choice Model help when:
- They have a serious mental health problem that lasts a long time
- A judge or doctor says they must get mental health care while they live at home
- A judge or doctor says they need extra mental health support while they live at home
- They were in a mental health program by court order and left it in the last year
- A court says they cannot understand what is happening in court or cannot help their lawyer
- A special court group that watched their case will stop in the next 6 months, or stopped in the last year
- The Department of Human Services says they can get help for gaining, disabilities, or learning problems, and they also need mental health help
- They might need any of the help above if they do not get support Choice Model services
Exceptional Needs Care Coordination
The Exceptional Needs Care Coordinator (ENCC) helps people who have big mental health needs. The ENCC talks with staff at the Oregon State Hospital and other mental health hospitals to plan when people leave and where they will go next.
The ENCC helps people who:
- Are staying at the Oregon State Hospital because a court said they must get treatment
- Have a court order for treatment and might need to go to the Oregon State Hospital
- Are at the Oregon State Hospital because their guarding (the person who makes legal choices for them) agreed
- Live in a treatment home and get help for a serious, long-lasting mental illness
- Live in a licensed adult foster home because they have a serious mental illness and need extra support
