Services provided include crisis intervention, screening for potential hospital admission, hospital diversion, respite placement and short term solution focused therapy as well as referral to appropriate community agencies
Services to adults 18 years and older suffering from persistent and severe mental illness, with impairments in functional ability that require service to provide stability and maintenance in the community and prevent decompensation that would require hospitalization. In addition, the illness impacts to ability to function independently causing a need for help with housing, shopping, social activities, and supportive relationships. A full range of services is provided to meet the need of each individual ranging from placement in an inpatient setting to periodic medication prescribing appointments.
Services are provided through:
Case coordination
Housing, Entitlement, Vocational Rehabilitation referral and coordination, Legal System advocacy and brokerage, other services related to restoring and maintaining community tenure, Medication Management, Transportation to appointments, Ongoing leisure and social activities, Intensive Stabilization Services, Assertive Outreach and Support, Phone support, office based as well as community-based sessions with counselors, Family Education and Support
